Where to get 401k information

All i received was a w-2 from my employer. I have a 401k plan and need to also report that information. I took a loan from it, but i am paying it back weekly. There is information on my w-2 about my 401k, but i don't know if i use the information from that or not. Where does it get entered on the tax form?I use turbotax deluxe. Last year i received a separate tax document for my 401k. My company changed 401k plans and now i don't know how to report it. Should i be getting a tax document on my 401k?

If you took a disbursement from your 401k, you'll get a 1099-R from the institution managing your fund. They have until the 30th to mail it out.

If you are paying back the loan it is not taxable...Only distributions are taxable and you would receive a 1099 for that -- but as long as you are still making payments it would not need to be reported on your taxes.
