Does an employee have a right to hold onto an employers 401k rollover request after the employee terminates his/her employee with the company

I received, completed, and returned my 401K rollover paperwork to my ex employee and over 6 weeks has gone by and the rollover is still not complete. The employer is holding the paperwork. Is this legal and is there a time frame as to when the paperwork must be turned in?
