What financial company manages sams club 401k

I worked there 6 years ago and realized i may have left a 401k there

This appears to be a tricky one, since Sam's Club is a subsidiary of Wal-Mart, Inc. The closest match I could get was Wal-Mart.Com USA, LLC 401K Plan. If this is the name of the plan, then the phone number is either 1-888-968-4015 or 1-650-837-5338. The address to write to is:

7000 Marina Boulevard
Brisbane, CA 94005-1815

The financial company that is trustee for the plan appears to be Merrill Lynch.

You can contact the HR manager at the Sam's Club store where you worked for information on how to access your 401-K.
